Mycoplasma genitalis

Mycoplasma genitalis is being recognized increasingly as a cause of urethritis, particularly in males accounting for 15-20% of nongonococccal urethritis.  The 2014 STD Treatment Guidelines from CDC note that doxycyclne is relatively poor at treating mycoplasma and azithromycin is recommended.  Doses of 1 gram effects cure in about 85% of cases, but may select for resistance. Azithromycin 500 mg initially followed by 250 mg daily for an additional 4 days might be slightly better but if resistance develops, the prolonged regimen is not effective.  In that case, Moxifloxacin 400 mg for 7, 10 or 14 days is recommended.  Sex partners should be managed according to the guidelines for non-gonococcal urethritis (or PID or cervicitis).
